• Immunization Requirements and Information

    State law (MS 121A.15) requires that each student in a Minnesota school be protected from childhood diseases by receiving immunizations. Parents/guardians may obtain a legal exemption for medical reasons or conscientious beliefs. All students new to the Saint Paul Public Schools (SPPS) must bring their immunization records or exemption record to school. Immunizations include Diphtheria, Pertussis and Tetanus (DPT); Polio; Measles, Mumps and Rubella (MMR); Hepatitis B; and Varicella (Chicken Pox). Immunization requirements vary according to grade.)

    Immunzation and/or Conscientious Objection Form for Kindergarten - 12th Grade or Child Care & Pre-Kindergarten students: These forms (or one with similar information) is required of every student in school. It provides dates of all immunizations received. It also provides exemption information. Print this form out to take to your next Health Care Provider (HCP) visit.

    Saint Paul-Ramsey County Public Health Immunization Clinic, 555 Cedar St. (10th and Cedar), offers low cost immunizations on the following days and times:

    Tuesday, 1:00-3:30 pm                                                         Evening clinics are held on Thursday, 4:00-7:00 pm

    Wednesday, 8:30-11:00 am and 2:00-4:00 pm

    Appointments are preferred; call 651-266-1234 to schedule.  Please bring your child's immunization record.
