American Indian Education Program

COMPLETED STUDENT ELIGIBILITY FORMS should be mailed, faxed or scanned then emailed to our program office at cynthia.s.johnson@spps.org or, they may be given to an Indian Education employee at your student's school. If you have further questions regarding eligibility, please call the Indian Ed Office at 651-293-5191. 

This form will become part of your child's school record and will NOT need to be completed every year. It will be maintained at SPPS Indian Education Office and information on the form will not be released without your written approval.

It is the purpose of this form to support the efforts of local educational agencies, Indian tribes and organizations, postsecondary institutions, and other entities to meet the unique educational and culturally related academic needs of American Indian and Alaska Native students, so that such students can meet the same challenging State student academic achievement standards as all other students are expected to meet.

This form will assist us in providing for your student the following: (1) meet the unique educational and culturally related academic needs of American Indians and Alaska Natives;(2) the education of Indian children and adults;(3) the training of Indian persons as educators and counselors, and in other professions serving Indian people; and (4) research, evaluation, data collection, and technical assistance.
