• Children’s Dental Services provides dental services in St Paul Public Schools.  Kids enrolled in the program receive dental treatment at their school including: cleanings, exams, X-rays, sealants, fillings, extractions, and crowns.  Once enough children from Benjamin E Mays School fill out the consent forms, a dental clinic will be held at Benjamin E Mays School.  Families with eligible students will be notified directly.

ALL STUDENTS ARE ELIGIBLE:   All forms of public and private insurance. No child is denied treatment due to an inability to pay. We assist families in applying for medical assistance and offer a sliding scale discount program for families ineligible for medical assistance.

TO SIGN UP:   Fill out a consent form for Children’s Dental Services for your child and return it to their teacher, the school nurse, or the front office staff.  Forms are good for 1 year from the date it is signed.

Please note that your child should not receive dental care from another provider if you choose Children's Dental Services school based dental care. Doing so risks over-treatment and insurance disputes.

Click on the consent to print, fill out, sign and return to school. Paper copies of the consents may be requested from the Mays Health Office 651-888-7606.
