Uniform Request Form

Please fill out your student(s) sizes and send it back to us so we can send uniforms home. We will be sending them home throughout the week 09/11-09/15 . If sizes don’t work please send back in a bag with a note on what sizes you do need. Each student will be given 2 uniforms and a sweatshirt.

All sweatshirts will be kept in the classroom on student’s chairs. When washed please send it back so students can have it here with them. Thanks for your patience and flexibility during this time as we work on making sure all students have uniforms.
