Take the Minnesota Community Care (Health Start) Assessment Survey

Minnesota Community Care (Health Start) requests your assistance to complete its Community Health Needs and Assets Assessment Survey (CHNAA). The CHNAA is completed every three years and shapes the vision, goals and strategy of their larger organization. Your completion of this survey will help Minnesota Community Care to better understand the strengths and needs of the communities they serve. The CHNAA also will help us to improve current and future partnerships, and resources that we provide to our patients and community. 

They are assessing the needs of current residents of Ramsey and Dakota County who attend, work, or have connections (including family members!) with Saint Paul Public Schools and are age 18 and older.

It is important that the survey participants reflect the patients and community we serve to create belonging and inclusion. Therefore, they ask demographic-related questions that are optional for people to complete. 

For your participation, you are eligible for a $10 e-gift card. If you want to receive an e-gift card, please provide Minnesota Community Care your name, email and phone number so they can send it to you once the timeframe closes to complete the survey.
