If you want the COVID-19 vaccine, please complete this form so that Saint Paul Ramsey County Public Health can reach out to you for vaccine opportunities in the near future.

It is important to complete the form to sign up for vaccine opportunities because the vaccine will soon be opened up to the general public. Once that happens, school staff will no longer be prioritized to receive the vaccine.

After completing the form, please remember to check your emails and answer the phone number you provided so that you can schedule your appointment.
