What is SmartCare?

The SmartCare Plan offers a lower premium by using a smaller alternative network compared to the Open Access network plans. You’ll have access to four SmartCare Clinics for primary care, plus the HealthPartners and Park Nicollet providers and facilities for additional care. For questions about which providers are in the SmartCare network, call HealthPartners at 952-883-5000.  

Instead of one doctor, at the SmartCare clinic you have a team of doctors, physician assistants and nurses who work together to stay up-to-date on your health.	When you call us, we’ll suggest the best type of visit. We’ll suggest a nurse, physician assistant or doctor. And we’ll let you know if you can video, message, call or visit the office based on your condition. Our hours are similar to what you’d find at a retail clinic.	You pay for the care you get, meaning a phone call with a physician assistant will cost you less than a visit in-person with a doctor.

If you enroll in a SmartCare plan you will have the same plan options - Copay (formerly Distinctions), HRA, or HSA, but you will need to get your primary care at one of four SmartCare Clinics:

If you need to see a specialist or need to undergo a medical procedure, all the HealthPartners and Park Nicollet providers and facilities are in the SmartCare network. You will also have access to nationwide coverage when you travel.
